B. Terms and Conditions for Claiming the Birthday Treat Free Dental Service

1. Eligibility

The birthday treat is available exclusively to active MO Card holders whose membership is valid on their birth month. The offer is non-transferable and applicable only to the cardholder.

2. Proof of Identity

Cardholders must present a valid government-issued ID or birth certificate that clearly states their date of birth. The MO Card must be presented along with proof of identity to claim the treat.

3. Booking and Availability

The free dental service must be booked in advance. Walk-ins are subject to availability and approval by the clinic. Bookings must be made within the cardholder’s birth month, and the appointment must also fall within the same month.

4. Service Coverage

The birthday treat includes only the specific free dental service listed in your MO Card benefits. Additional treatments are subject to regular rates.

5. Participating Clinics

The free dental services can only be availed at affiliated clinics of MO Cards. Please confirm the list of participating clinics beforehand.

6. Limitations

The birthday treat can be claimed once per year by the card legitimate owner. Rescheduling of appointments is allowed but must remain within the birth month. Missed appointments without prior notice may result in the forfeiture of the birthday treat.

7. Disputes and Clarifications

Any disputes regarding the eligibility or redemption of the birthday treat will be resolved by MO Cards in consultation with the affiliated clinic.

By availing of the birthday treat, the cardholder agrees to these terms and conditions. For inquiries, please contact our customer service team.

C.TERMS AND CONDITIONS (for unlocking other free dental services)

By proceeding with registration, I acknowledge that I have read and understood the following Terms and Conditions and agree to abide by them.

As part of our advocacy to provide access to oral healthcare in the Philippines, we offer community dental services through our affiliated clinics. In order to claim the free dental services under this program, a one-time registration fee of ₱499 is required. Please note that this fee carries no further obligations. Upon successful registration, the cardholder is entitled to the following free dental services, honored in our affiliated clinics nationwide:

(1) Mild Oral Prophylaxis (Cleaning)

(1) Simple Tooth Extraction

(1) Light Cure Filling (Small Surface Restoration)

(1) Oral Fluoride Treatment

These services are intended to support individuals with mild dental cases, allowing them to save money or reserve financial resources for more severe dental concerns that may require advanced care for themselves or their family members.

Terms of Use: All free dental services, except dental check-ups and discounts, are non-transferable and may only be availed by the registered cardholder. If the cardholder's condition is deemed beyond mild (i.e., a complex case), they may opt to use the 20% discount benefit instead and save their free service for a future qualifying condition.

Free service access is valid for one (1) year from the date of registration and may be renewed annually for ₱499. Renewal is optional. However, failure to renew will result in deactivation of dental access, and the cardholder’s data will be removed from our affiliated clinic systems. The card will no longer be accepted for free service claims.

Lifetime Use of Card: The card itself is lifetime-valid. Even after dental access has been deactivated, the card may still be used as a personal or business contact card. They may simply log in again, register, and create a new profile. We highly encourage maintaining your annual renewal to maximize benefits and support our mission of sustaining accessible dental care in the country.
